Working with primary care

We work with GP practices and primary care networks in the South West to understand what their teams need. The South West Collaborative and Models of Care Portal are two examples of our work to support sustainability and innovation in primary care in the region.

South West Collaborative

The South West Collaborative is a joint initiative by the South West Academic Health Science Network (SW AHSN) and NHS England to support primary care teams to create ‘headspace’ in order to deliver ideas for change on the frontline.

Supported by a small team of clinicians, managers, and analysts, we are working with 12 practices in the South West across four core themes:

  • Data visualisation of flow and demand
  • Social prescribing
  • Adopting innovation
  • Workforce redesign

Together we aim to create a learning network that empowers teams working at practice level to do things differently. Virtual meetings and face-to-face conversations are driven by the desire to be innovative, creative, and to have fun.

Models of Care Portal

The Models of Care Portal is an initiative developed by the SW AHSN in collaboration with NHS England (South West) as part of their programme to support workforce sustainability.

The portal provides users with easy access to case studies on what is and isn't working as well as what has been tried in primary care and across the health and care system. The portal is free to use and supports shared learning within the context of the whole system.

The portal also features:

  • A private forum for registered users.
  • Access to resources and funding opportunities.
  • A shared project space where users can register a project and invite others to collaborate.
  • Workforce planning tools.
