To spread innovation in health and care in the South West, we need leaders with the skills to unleash change.

Join our second Spread Academy on personalised care!
Together with the Billions Institute and NHS England & Improvement, we are offering teams of 3 to 6 people working in health and care across the South West the opportunity to join our second Spread Academy on personalised care, to be held at Dartington Hall in Totnes, Devon, from 24-26 September 2019.
Led in partnership with NHS England and Improvement, the three-day programme will explore how to lead changes to the health and care system that empower individuals to become integrators of their own care. Successful teams will also be invited to an ‘all teach, all learn’ day on 17 October 2019 (location TBC).
How do I apply?
All teams who apply must be working to implement universal personalised care and must include someone with lived experience of health care and support, either directly or as a carer.
You may be working in local areas of different sizes, including Integrated Care System/Sustainability and Transformation Partnership footprints, Primary Care Networks, local neighbourhoods, or regional networks.
Multi-disciplinary teams are encouraged to apply, preferably from organisations representing multiple sectors such as the NHS (commissioners and providers), local authority, or the voluntary and community sector.
Please complete a single application for your team, including personal statements from each team member. Each application should have one lead applicant who will act as the single point of contact for the application process. This applicant will also need to provide the name and contact details of a senior person (at CEO or director level) from their local system who can endorse your team’s application.

The Spread Academy is an initiative of the SW AHSN to support large-scale change in the health and care system that improve our economy and patients’ lives. The course in September 2019 is commissioned by NHS England and is focused on the spread of initiatives to support Personalised Care.
