The South West Collaborative is an evolving initiative bringing together data, insight and innovation to meet primary care’s biggest challenges.
Developed by the South West Academic Health Science Network (SW AHSN) in alliance with other organisations, the collaborative has been working since early 2019 to understand the challenges individuals and primary care teams face as well as their aspirations for change and improvement. With primary care teams pressured for time, our key focus is to create the ‘headspace’ teams need to allow change to happen.
Supported by a team of regional clinicians, managers and analysts, we are working with a growing number of practices in the South West of England on data visualisation of flow and demand, social prescribing, adopting innovation, and workforce redesign.
We have aligned with the National Association of Primary Care (NAPC) to explore the potential for effective population health delivery and can connect with other key providers of support such as Health Education England, Public Health England and NHS England’s Integrated Personal Commissioning programme.
Together we are creating a learning network that empowers teams working at practice level to do things differently. Virtual meetings and face-to-face conversations are driven by the desire to be innovative, creative, and to have fun.
How can the South West Collaborative help you?
Our programme is committed to supporting primary care networks across the South West region to generate change at pace and scale.
We work with individuals and teams in three ways:
- We listen. By understanding the challenges within your practice, our experts can help to provide the headspace you need to explore your ideas, including primary care network set-up, social prescribing and leadership development.
- We visualise. We use the latest digital tools to transform your practice data into visualisations that can highlight opportunities to optimise how your team works.
- We connect. We support collaborative working by connecting you to like-minded primary care professionals, groups and organisations across your network to help your practice to flourish.
The collaborative offers teams a space to share ideas and progress across primary care on Models of Care, an online platform developed by the SW AHSN in partnership with NHS England. Explore case studies and resources, build your own networks, and create your own collaboration space to contribute to progress.
