The national Patient Safety Collaborative (PSC) programme aims to support and encourage a culture of safety, continuous learning and improvement across the health and care system, helping to reduce the risk of harm and make care safer for all.

The PSC is a joint initiative between NHS Improvement and the AHSN Network. The programme, which was set up in 2013, is funded and nationally coordinated by NHS Improvement, and organised and delivered locally by regional Academic Health Science Networks (AHSNs).

The collaborative has three national workstreams focussing on growing a robust safety culture, developing maternal and neonatal patient safety, and enhancing outcomes for deteriorating patients.

In addition, a range of local programmes has been developed to improve safety and promote the wellbeing of our local population.

The South West Patient Safety Collaborative (SW PSC) is hosted by the South West Academic Health Science Network (SW AHSN) and exists to create a culture of continuous quality improvement across the South West. The collaborative works with health organisations including Clinical Commissioning Groups, acute trusts, mental health trusts and secondary care providers across Cornwall and the Isles of Scilly, Devon and Somerset. The SW PSC provides a range of quality improvement and patient safety events and projects and works continuously to improve patient safety across the whole system.
